State of Florida investigators with the Agency for Healthcare Administration conducted an unannounced licensure recertification survey and investigation at Village Place Health and Rehabilitation Center between May 2, 2016 and May 5, 2016. This facility is a 104 bed nursing home located at 2370 Harbor Boulevard, Port Charlotte, Florida 33952, and is run by the for-profit nursing home chain, Greystone Healthcare. This nursing home has a history of paying fines to the State of Florida and had its license downgraded to conditional status during 2011.
Investigators interviewed residents, staff members and administration during their visit and cited Village Place Health and Rehabilitation Center as operating in violation of the federal Nursing Home Reform Act in connection with the care and services provided to actual residents living on the premises or there for the purpose of receiving rehabilitative therapy. The results of the investigation and survey were recently made public. The survey and citation of deficiencies concerned the following topics and issues:
F 241 The failure to promote care for residents in a manner and in an environment that maintains or enhances each resident’s dignity and respect.
F 246 The failure to ensure that each resident has the right to receive reasonable accommodations of individual needs and preferences.
F 248 The failure to provide an ongoing program of activities designed to meet resident needs.
F 309 The failure to provide the necessary care and services to one or more residents to attain or maintain their health and well-being.
F 329 The failure to ensure that each resident’s drug regimen is free from unnecessary drugs.
F 356 The failure to post daily nursing staffing information for the facility.
F 441 The failure to establish and maintain an Infection Control Program designed to provide a safe, sanitary and comfortable environment for each resident.
F 514 The failure to maintain clinical records on each resident in accordance with accepted professional standards and practices.
Furthermore, Village Place Health and Rehabilitation Center was cited for violating Florida law because, according to Florida officials, it failed to meet the standard that requires nursing homes to provide residents with adequate and appropriate healthcare.
